Thanks to all who replied, including offlist direct replies.
Understanding what is happening doesn't indicate to me why they got
the client hold state, but in both cases, the customer is handling the
domain directly with the registrar so it is likely something the
customers did.  It's aggravating that I cannot make the necessary
change but that's a required barrier, obviously, to prevent domain
hijacking.  In both cases, my advice to tech support was that the
customer needed to contact the registrar directly, themselves.
Originally I felt like maybe I could have been doing more, hence my
query to this list.
