[as112-ops] New AS112 Node and Some Documentation Fixes

Greetings,

We have spun up a new AS112 on our IXP:
Who: Fremont Cabal Internet Exchange
Where: Fremont, CA, US
Unicast Addresses: 23.152.160.9, 2620:13b:0:1000::9
Transit AS: 7034

While deploying this node, I wrote it as an ansible playbook, which I
suspect others might find useful for deploying nodes and/or checking
our work: https://github.com/PhirePhly/as112-ixp-ansible

I'd also like to point out that I suspect there's a typo on the
website here: https://www.as112.net/well-known.html

It currently lists the well-known addresses as:
192.175.48.1 2620:4f:8000::1
192.175.48.6 2620:4f:8000::6
192.175.48.42 2620:4f:8000::42
192.31.196.0/24 2001:4:112::/48

I believe these addresses should be listed as:
192.175.48.1/24 2620:4f:8000::1/48
192.175.48.6/24 2620:4f:8000::6/48
192.175.48.42/24 2620:4f:8000::42/48
192.31.196.1/24 2001:4:112::1/48
